      <description>Jeff: Hello from Iowa. What was the explanation at to why no one inserted the chest tube? Nursing shortage? No clear lines of authority? Why didn't the attending physician know his order for the chest tube had not been executed? Was there a product issue with the breathing machine? What is the make and model of the breathing machine? Perhaps this is something medical professionals need to know about. Thanks for reporting this this news item. Perhaps Judith Livingston could be interviewed or would be willing to write a post in follow up. There is more to this than meets the eye.</description>
